About the role

The Hospice Social Worker assesses the psychosocial needs of patients and families, providing resource support, counseling, and crisis intervention in collaboration with interdisciplinary teams.

Responsibilities

  • Conduct psychosocial assessments to identify emotional, social, and environmental strengths and challenges.
  • Develop and implement a bereavement care plan tailored to family and caregiver needs.
  • Interpret and communicate patients' and families' faith and cultural traditions, considering their impact on care.
  • Document assessments, care plans, interventions, and changes accurately within electronic medical records.
  • Facilitate patient and family decision-making, respecting cultural beliefs and values.
  • Address needs related to imminent death, including end-of-life planning and spiritual support.
  • Assist with advanced directives and referrals for resources such as transportation, food, housing, and elder abuse concerns.
  • Ensure compliance with Medicare Hospice benefits and related documentation.

Qualifications

  • Master’s Degree in Social Work
  • Licensure as an LSW or LCSW
  • Valid driver’s license
  • Experience in hospice care
  • Strong assessment, communication, and culturally sensitive care skills
