About the role
The Hospice Social Worker assesses the psychosocial needs of patients and families, providing resource support, counseling, and crisis intervention in collaboration with interdisciplinary teams.
Responsibilities
- Conduct psychosocial assessments to identify emotional, social, and environmental strengths and challenges.
- Develop and implement a bereavement care plan tailored to family and caregiver needs.
- Interpret and communicate patients' and families' faith and cultural traditions, considering their impact on care.
- Document assessments, care plans, interventions, and changes accurately within electronic medical records.
- Facilitate patient and family decision-making, respecting cultural beliefs and values.
- Address needs related to imminent death, including end-of-life planning and spiritual support.
- Assist with advanced directives and referrals for resources such as transportation, food, housing, and elder abuse concerns.
- Ensure compliance with Medicare Hospice benefits and related documentation.
Qualifications
- Master’s Degree in Social Work
- Licensure as an LSW or LCSW
- Valid driver’s license
- Experience in hospice care
- Strong assessment, communication, and culturally sensitive care skills
