Social Worker (LSW/LCSW) - Hospice

About the role

The Hospice Social Worker assesses the psychosocial needs of patients and families through comprehensive evaluations. Provides interventions including resource support, short-term counseling, crisis intervention, and collaboration with interdisciplinary teams to enhance patient care. Develops and implements care plans aimed at strengthening family systems, maximizing community resource utilization, supporting cultural and spiritual beliefs, and promoting dignity and coping skills. Documents assessments, interventions, and care plans accurately and timely in electronic medical records. Addresses end-of-life needs, including explaining hospice benefits, assisting with advanced directives, and supporting patient decisions. Facilitates access to services such as transportation, financial aid, and elder abuse reporting, ensuring holistic support for patients and families. Ensures compliance with policies and practices sensitive to palliative care philosophy, demonstrating understanding of hospice regulations and patient-centered care.

Qualifications

• Master’s Degree in Social Work
• Illinois licensure (LSW or LCSW)
• Valid driver’s license
• Relevant hospice experience
