(LSW) Social Worker Hospice Home Care

Responsibilities

• Provide emotional and logistical support to patients and their loved ones during their final days.
• Affirm patients' wishes regarding finances, funeral arrangements, and other matters.
• Navigate the grieving process for patients and their families.
• Grant end-of-life wishes and listen to patients' stories at their bedside.
• Care for hospice patients in their homes or assisted living/skilled nursing facilities.
• Meet with interdisciplinary team members in the office about once a week.

Requirements

• Ability to travel as required.
• Thorough knowledge of community resources or willingness to acquire such knowledge.
• Knowledge of key psychosocial issues related to terminal illness and their impact on patients and families.
• Excellent interpersonal and communication skills.
• Capacity to work effectively as part of an interdisciplinary team.
• Experience working with terminally ill patients and their families.
• Empathy and compassion towards seriously ill patients and their families.
• Licensure as a Social Worker, typically a Master's of Social Work (MSW).
• At least one year of experience in a healthcare setting, preferably in social work.

Qualifications

• Masters of Social Work from an accredited school.
• One year of experience in a healthcare setting, preferably in social work.
• Preferred experience with terminally ill patients and their families.
• License and/or certification required by state law.
