Social Worker (LSW/LCSW) - Home Services

Overview: This role involves assessing the psychosocial needs of patients and families, providing resource support, counseling, and crisis intervention as part of an interdisciplinary team. Qualifications: Requires a Master’s Degree in Social Work, licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois, a valid driver’s license, proof of auto insurance, and at least one year of hospice experience. Key Responsibilities Conduct psychosocial assessments to identify emotional, social, and environmental strengths and challenges.Develop bereavement care plans and interpret cultural and spiritual beliefs impacting care.Educate patients and families, facilitate communication, and document assessments and interventions accurately.Implement social work plans to enhance family support, resource utilization, and patient dignity.Support patient decision-making, address end-of-life needs, and educate on advanced directives.Assist with transportation, housing, financial referrals, application assistance, and elder abuse reporting. Skills: Ability to work collaboratively, sensitive to patient needs, knowledgeable about hospice and palliative care philosophy, and proficient in documentation and community resource navigation.
