Social Worker (LSW/LCSW) - Home Health

Overview

The Home Services Social Worker assesses psychosocial needs of patients and families, providing resource support, short-term counseling, and crisis intervention in collaboration with interdisciplinary teams. The role requires hospice experience of over one year and involves developing care plans to enhance family strength, maximize benefits, and support cultural and spiritual preferences.

Responsibilities

• Conducting psychosocial assessments of patients and families
• Developing and implementing social work care plans
• Supporting patient and family decision-making and cultural needs
• Addressing barriers to care and facilitating resource access
• Completing timely documentation and coordinating with interdisciplinary teams
• Assisting with advanced directives and end-of-life planning
• Making referrals for community resources and elder abuse concerns
• Facilitating self-determined care decisions and addressing imminent death indicators
• Assisting with transportation, referrals for food, utilities, applications for assistance programs, and elder abuse referrals

Qualifications

• Master's Degree in Social Work
• Licensure as an LSW or LCSW through the Illinois Department of Financial and Professional Regulation
• Valid driver's license and auto insurance
• Over one year of hospice experience
• Strong teamwork skills
• Understanding of palliative care philosophy
