About the role

The Home Services Social Worker assesses the psychosocial needs of patients and families, providing resource support, short-term counseling, and crisis intervention as part of an interdisciplinary team. The role requires Hospice experience of 1+ years.

Qualifications

• Master's Degree in Social Work
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois
• Valid Driver's License and proof of auto insurance
• Experience in hospice care

Responsibilities

• Conduct psychosocial assessments to identify emotional, social, and environmental issues
• Develop bereavement and social work plans, ensuring culturally sensitive communication
• Educate patients and families, addressing barriers to care and explaining advance directives
• Coordinate resources, including transportation, financial assistance, and community referrals
• Document assessments, interventions, and care plans accurately and timely
• Support patient self-determination and address end-of-life needs
• Collaborate with the interdisciplinary team to optimize patient care and maximize benefits
• Address indicators of imminent death and assist with related documentation and procedures
