About the role

The Home Services Social Worker provides psychosocial assessments and interventions for patients and families, addressing emotional, social, and environmental needs related to illness and end-of-life care.

Responsibilities

• Developing bereavement plans
• Interpreting cultural and spiritual beliefs
• Educating patients and families
• Documenting assessments and interventions
• Collaborating with interdisciplinary teams to maximize patient support and dignity

Requirements

• Master’s degree in Social Work
• Licensure as LSW or LCSW in Illinois
• One or more years of hospice experience
• A valid driver’s license
• Proof of auto insurance
