About the Role
The Home Services Social Worker assesses the psychosocial needs of patients and families, providing resource support, short-term counseling, and crisis intervention in collaboration with interdisciplinary teams. The role involves developing care plans, facilitating communication of cultural and spiritual beliefs, and supporting patient self-determination and end-of-life decisions.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Develop bereavement and social work plans.
- Document interventions and evaluate their effectiveness.
- Assist with barriers to transportation, food, utilities, and access to home services.
- Make referrals for community resources, legal directives, and elder abuse prevention.
- Support patients with advance directives and hospice benefit processes.
Requirements
- Master’s degree in Social Work.
- Licensure as an LSW or LCSW.
- Valid driver’s license and auto insurance.
- At least one year of hospice experience.
- Ability to work collaboratively in interdisciplinary teams.
- Demonstrated cultural sensitivity and alignment with palliative care philosophy.
