Social Worker LCSW - Social Work Services

About the role

We are seeking a part-time Licensed Clinical Social Worker (LCSW) to provide comprehensive biopsychosocial and spiritual assessments, diagnoses, treatments, and advocacy for diverse patient populations.

Responsibilities

• Collaborate with patients and support systems to develop and implement evidence-based, person-centered care plans across the continuum of care.
• Perform assessments and interventions while respecting patient self-determination and reducing health disparities.
• Work closely with interdisciplinary teams across inpatient and outpatient settings.
• Adhere to NASW standards, ethics, and organizational mission.
• Conduct assessments, develop care plans, advocate for patients, and coordinate with healthcare teams to promote holistic, equitable care.

Requirements

• Master's Degree in Social Work from an accredited or candidate CSWE program.
• Licensed in Oregon, Washington, or other relevant states upon hire.
• Minimum of 2 years relevant social work experience, including CSWA experience preferred.
