This is a full-time, 1.0 FTE position located at East Madison Hospital in Madison, WI.
About the role
Join the #1 hospital in Wisconsin as a Social Worker in the Emergency Department. You will provide psychosocial assessment, intervention, and support to patients and their families, addressing a wide range of needs including trauma response, acute grief support, psychiatric dispositions, and resource connection.
Responsibilities
- Provide psychosocial assessment, intervention, and support to Emergency Department patients and their social supports.
- Respond and intervene in Trauma activations, offering acute grief support.
- Assist with psychiatric dispositions and support patient needs, including transportation, mental health, housing/homelessness, in-home support, hospice, and insurance programs.
- Serve as a support to patients, families, and the ED team in cases of abuse and neglect concerns.
- Connect patients with community programs and resources to resolve psychosocial risks and barriers to discharge.
- Intervene with support systems caring for patients with complex medical needs, providing crisis management and education.
- Support the Social Work team through case consultation, coverage support, and participation in improvement projects.
- Optionally participate in clinical supervision to advance to advanced clinical licensure status.
- Optionally participate in Social Work staffing models to address needs across the care continuum.
- Utilize Social Work interventions to enhance patient and family functioning.
- Collaborate with the multidisciplinary Emergency Department team to develop patient goals, monitor progress, address barriers, and evaluate outcomes of individualized care plans.
Requirements
- Master’s Degree in Social Work (required).
- 2 years of prior hospital-based social work experience (preferred).
- 2 years of prior hospital or community-based social work experience if supporting the HIV Clinic (preferred).
- 2 years of prior hospital or clinic-based pediatric experience if supporting American Family Children’s Hospital (AFCH) (preferred).
Qualifications
- Certified Advanced Practice Social Worker (CAPSW) in the state of Wisconsin within 90 days (required).
- Certified Independent Social Worker (CISW) in the state of Wisconsin within 90 days (required).
- Licensed Clinical Social Worker (LCSW) in the state of Wisconsin within 90 days (required).
Schedule
Eight-hour shifts, Monday – Friday between the hours of 3:00 pm – 11:30 pm. Hours may vary based on operational needs of the department.
