About the role

A Social Worker - MSW (Inpatient) is a crucial member of the healthcare team, providing comprehensive support and services to patients and their families.

Schedule

This position works four 10-hour shifts per week.

Responsibilities

  • Conduct thorough assessments of patients' mental, emotional, and social well-being to evaluate their needs and identify any barriers to their overall functioning, including support systems, coping mechanisms, mental health conditions, and psychosocial factors.
  • Collaborate with providers, patients, their families, and the healthcare team on individualized care plans that align with patients' goals, preferences, and values.
  • Connect patients and their families with community resources, mental health services, support groups, and other relevant programs, including referrals for psychiatric evaluations and medication management.
  • Continuously monitor and evaluate the progress of patients' care plans to assess their response to interventions, measure outcomes, and make necessary adjustments.
  • Advocate for patients' needs within the healthcare system and the community, ensuring access to appropriate treatment and addressing barriers.
  • Engage in active listening to understand patients' experiences, concerns, and emotions, providing a safe and non-judgmental space for expression.
  • Recognize and respect the unique values, beliefs, and cultural backgrounds of each patient, tailoring interventions accordingly.
  • Build trust-based relationships with patients and their families through empathy, confidentiality, and professionalism.
  • Utilize risk segmentation strategies to identify patients requiring additional support or interventions based on medical, psychological, and social risk factors.
  • Carry out specific tasks or interventions as delegated by the healthcare team, such as coordinating referrals and conducting follow-up assessments.
  • Coordinate care across different providers and settings to ensure seamless transitions and continuity of care.
  • Communicate with payers, such as insurance companies or government agencies, for authorization of services, reimbursement, or documentation requirements.

Requirements

  • Master's Degree in Social Work
  • Two (2) years of health care experience strongly preferred

Pay

$33.37 - $47.97 per hour (Hiring Range)

Job Details

This is a full-time, onsite position located in Raleigh, NC, with standard hours of 40.00 per week. The role is exempt from overtime.
