Responsibilities
- Conduct thorough assessments of patients' mental, emotional, and social well-being to evaluate their needs and identify any barriers to their overall functioning.
- Avoidance of barriers to care and ensuring access to appropriate treatment.
- Collaborate with providers, patients, their families, and the healthcare team on individualized care plans that align with patients' goals, preferences, and values.
- Connect patients and their families with community resources, mental health services, support groups, and other relevant programs.
- Continuously monitor and evaluate the progress of patients' care plans to assess their response to interventions, measure outcomes, and make necessary adjustments.
- Advocate for patients' needs within the healthcare system and the community.
- Engage in active listening to understand patients' experiences, concerns, and emotions.
- Recognize and respect the unique values, beliefs, and cultural backgrounds of each patient.
- Tailor interventions and support to align with their individual needs and preferences, taking into account their cultural, social, and spiritual backgrounds.
- Build trust-based relationships with patients and their families by demonstrating empathy, confidentiality, and professionalism.
- Foster open communication, collaboration, and a therapeutic alliance.
- Utilize risk segmentation strategies to identify patients who may require additional support or interventions based on their medical, psychological, and social risk factors.
- Carry out specific tasks or interventions as delegated by the healthcare team.
- Collaborate with other members of the healthcare team to carry out delegated work, such as coordinating referrals, conducting follow-up assessments, and providing ongoing support to patients and their families.
- Carefully coordinate care across different providers and settings to ensure seamless transitions and continuity of care.
- Facilitate communication, share information, and advocate for the patient's needs.
- Communicate with payers, such as insurance companies or government agencies, to provide necessary information for authorization of services, reimbursement, or other documentation requirements.
Requirements
Education Requirements: Master's Degree in Social Work
Licensure/Certification Requirements: None required
Professional Experience Requirements: Two (2) years health care experience strongly preferred
