Social Worker - Emergency Department

About the role

The Emergency Department Social Worker provides comprehensive psycho-social assessment, management, and support to at-risk patients and families at Upstate Medical University Hospital Community campus. This role involves understanding social determinants, population health, and drivers of utilization.

Responsibilities

• Conduct psycho-social assessments, counseling, intervention, planning, and referrals to address patient and family needs.
• Initiate and implement advance directives.
• Actively manage patients through collaboration, coordination, monitoring, and evaluating options to meet individual health needs.
• Round on assigned patients with the multidisciplinary team.
• Participate in quality meetings for the assigned unit.
• Utilize communication skills and knowledge of community resources to support patients.
• Document all interventions and participate in departmental meetings, in-services, education, and quality improvement activities.
• Perform other relevant duties, including coverage for other social workers and back-up responsibilities.

Requirements

• MSW from a Social Work program accredited by the Council on Social Work Education (CSWE).
• L.M.S.W., L.C.S.W., or eligible; 3 years of relevant post-master's experience (preferably in a hospital and/or healthcare environment).
• New M.S.W. graduates who are L.M.S.W. eligible may be considered for an entry-level (SL2) position after obtaining a Limited Permit.
• Must obtain L.M.S.W. within 12 months of receipt of limited permit.

Schedule

Evening shifts with weekend rotation. Occasional on-call and holiday coverage based on operational needs.
