About the role
The Emergency Department Social Worker provides comprehensive psycho-social assessment, management, and support to at-risk patients and families at Upstate Medical University Hospital Community campus. This role involves understanding social determinants, population health, and drivers of utilization.
Responsibilities
- Conduct psycho-social assessments, counseling, intervention, planning, and referrals to address patient and family needs.
- Initiate and implement advance directives.
- Actively manage patients through collaboration, coordination, monitoring, and evaluating options to meet individual health needs.
- Round on assigned patients with the multidisciplinary team.
- Participate in quality meetings for the assigned unit.
- Utilize communication skills and knowledge of community resources to support patients.
- Document all interventions and participate in departmental meetings, in-services, education, and quality improvement activities.
- Perform other relevant duties, including coverage for other social workers and back-up responsibilities.
Requirements
- MSW from a Social Work program accredited by the Council on Social Work Education (CSWE).
- L.M.S.W., L.C.S.W., or eligible; 3 years of relevant post-master's experience (preferably in a hospital and/or healthcare environment).
- New M.S.W. graduates who are L.M.S.W. eligible may be considered for an entry-level (SL2) position after obtaining a Limited Permit.
- Must obtain L.M.S.W. within 12 months of receipt of limited permit.
Schedule
Evening shifts with weekend rotation. Occasional on-call and holiday coverage based on operational needs.
