Basic Function
The Social Services Care Coordinator facilitates and coordinates substance use disorder treatment clients' ancillary needs, and documents the services provided.
Responsibilities
- Ensure clients’ TB tests are completed within 7 days of admission.
- Apply clients for state disability benefits, as needed.
- Apply clients for Medi-Cal benefits.
- Arrange for client transportation to medical facilities.
- Transport clients as needed, i.e. to court, DPSS, etc.
- Aid clients to enroll with a primary care physician / medical group.
- Schedule new clients for physical examinations.
- Maintain professional rapport with all networking agencies.
- Act as liaison between clients and CDWs concerning medical issues.
- Obtain medication approval from Social Services Manager or Director of Nursing.
- Monitor clients’ ongoing medical issues and clients’ medications.
- Review all medical treatment forms when clients return to the facility.
- Report all client illnesses to Director of Nursing.
- Alert the Director of Nursing about all client accidents, injuries and medical issues.
- Update and ensure accuracy of clients’ medication records.
- Document Care Coordination in EHR (currently PCNX).
- Perform other related duties as assigned by the Social Services Manager.
Skills
- Must be able to type a minimum of 40 wpm.
- Must have excellent computer skills.
- Must have strong organizational, follow-up skills and time management skills.
- Must be proficient in Microsoft Excel.
- Must be able to work well within a team structure.
- Must have excellent oral and written communication skills.
Education / Training
- If the individual is a registered addictions counselor, they must be enrolled in an academic Addiction Studies Program.
Licenses / Certifications
- Position requires that individual be registered and / or certified with one of the state-approved addiction counselor certification entities (CADTP, CAADE, or CCAPP).
Special Conditions
- Must have a valid California driver's license.
- Must be willing to undergo random drug screenings.
Working Conditions
- Occasional ascending and descending stairs.
- Frequent sitting for prolonged periods.
- Occasional crouching below the waist and / or above the shoulders.
- Occasional operation of motor vehicles.
- Occasional work may be performed at various locations across Los Angeles County.
