About the role
The Social Services Coordinator at Bishop Spencer Place is responsible for assessing and addressing the psychological, social, emotional, cultural, and financial needs of residents that impact their health and recovery. This position involves providing professional social work services, utilizing supportive counseling and clinical knowledge, and collaborating with the health care team to develop transition plans and address patient needs.
Responsibilities
- Assess the psychological, social, emotional, cultural, and financial needs of residents impacting their health and recovery.
- Provide professional social work services including supportive counseling and clinical knowledge.
- Collaborate with the health care team in developing a transition plan and addressing needs in a timely manner.
- Participate in program planning and development to meet the needs of customers within the health care system and community.
- Work with the multidisciplinary team to provide services that impact patient health and recovery.
- Assist the Social Work team and patients with discharge arrangements, ensuring they have the correct services in place prior to discharge.
- Assist patients in making transportation arrangements.
- Participate in treatment team meetings.
Requirements
Applicants must have a Bachelor's degree and less than one year of relevant experience in senior living. Previous experience in senior living is preferred.
Qualifications
Bachelor's degree required.
Skills
Proficiency in social work principles, excellent communication skills, ability to work collaboratively with a multidisciplinary team, and strong organizational skills are essential.

Full-time, Monday through Friday, 8 a.m. - 5 p.m.
