Social Services Coordinator
About the role
Manages legal issues related to adoptions, guardianship, and advance directives in collaboration with legal counsel.
Educates patients and families on available resources and coping strategies for dealing with diagnosis, treatment, and prognosis.
Collaborates with hospital and community agencies to secure needed services and resources for patients and caregivers.
Collaborates with RN Case Managers to develop and implement post-discharge plans tailored to each patient's needs.
Interviews patients and families to assess psycho-social needs and evaluate support systems.
Collaborates with the healthcare interdisciplinary team to address barriers impacting discharge plans.
Identifies and initiates the most efficient discharge plan to expedite patient transition to the next level of care.
Facilitates communication between patients, families, and healthcare providers to ensure understanding of discharge plans and available resources.
Responsibilities

Qualifications
- Bachelor's degree in Social Work
- Acute hospital experience preferred
- Basic Life Support - CPR Cert (BLS) required
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) preferred
