Qualifications
- A bachelor’s degree in social work, gerontology, or a related field within health and human services including, but not limited to, sociology, special education, rehabilitation, counseling and psychology.
- Ability to obtain a Licensed Social Worker (LSW).
- Minimum of one year of supervised social work experience in a health care setting working directly with individuals.
- Interpersonal and communication skills that identify the ability to positively and effectively communicate with others while completing work accurately and timely.
- A strong commitment to helping others and consistently treating them with empathy, respect, patience, and discretion.
Key Responsibilities
- Assess residents’ psychosocial needs and complete social histories.
- Complete assigned MDS sections accurately and on time.
- Document care plans, progress, and follow-up per regulations.
- Provide counseling and emotional support to residents and families.
- Support residents and families with adjustment, rights, and orientation.
- Assist with discharge planning and care transitions.
- Coordinate referrals and act as a liaison with community resources.
- Partner with the care team to enhance quality of life and outcomes.
- Plan and support resident, family, and community engagement activities.
- Promote facility programs and services to referral partners.
