Responsibilities

Aid in the pre-admission and admission processes, serving as backup to Admissions Department personnel.
Conduct tours and sign necessary paperwork for clients and families.
Interview clients and families to assess financial status and eligibility for Medicaid assistance.
Help families apply for Medicaid (ICP) approval and attend annual reviews if needed.
Track and report the status of Medicaid applications/approvals for the facility bookkeeper.
Support Social Services Director during interviews of residents and families, gathering relevant social histories.
Offer one-on-one counseling and group discussions to assist residents and families in adjusting to facility life.
Plan and coordinate post-discharge services and follow-up care for residents and families.
Maintain open communication with residents, families, healthcare professionals, and facility staff.
Provide assistance in any situation as needed, including access to protected health information.

Qualifications

High school diploma or equivalent.
Bachelor’s degree in Social Work or Human Services preferred.
One year of supervised social work experience in a healthcare setting, working directly with individuals.
Experience or knowledge of Institutional Medicaid Program (ICP) qualifications and application process preferred.
Ability to work with a variety of people in different settings, including community and healthcare environments.
Capable of making decisions based on evaluations and interpretations of clients' case histories.
