Responsibilities

  • Aid in the pre-admission and admission processes, serving as backup to Admissions Department personnel.
  • Conduct tours and sign necessary paperwork for clients and families.
  • Interview clients and families to assess financial status and eligibility for Medicaid assistance.
  • Help families apply for Medicaid (ICP) approval and attend annual reviews if needed.
  • Track and report the status of Medicaid applications/approvals for the facility bookkeeper.
  • Support Social Services Director during interviews of residents and families, gathering relevant social histories.
  • Offer one-on-one counseling and group discussions to assist residents and families in adjusting to facility life.
  • Plan and coordinate post-discharge services and follow-up care for residents and families.
  • Maintain open communication with residents, families, healthcare professionals, and facility staff.
  • Provide assistance in any situation as needed, including access to protected health information.

Qualifications

  • High school diploma or equivalent.
  • Bachelor’s degree in Social Work or Human Services preferred.
  • One year of supervised social work experience in a healthcare setting, working directly with individuals.
  • Experience or knowledge of Institutional Medicaid Program (ICP) qualifications and application process preferred.
  • Ability to work with a variety of people in different settings, including community and healthcare environments.
  • Capable of making decisions based on evaluations and interpretations of clients' case histories.
