About the role
Reporting to the Director of Social Services or Program Director, the Social Services Supervisor oversees the clinical and case management functions of the Social Service Department, ensuring the ongoing best practice approach of program functioning and compliance with regulatory agency policies and procedures.
Responsibilities
- Program/Facilities Supervises day to day operations and clinical staff in the absence of the Director of Social Services / Program Director.
- Supervises and provides leadership and training to social service team members.
- Provides administrative and programmatic supervision to case management staff.
- Conducts probationary and annual performance reviews for staff.
- Affords support and guidance to case managers in the execution of all housing related tasks.
- Makes weekly supervision with all direct reports.
- Screens and intakes clients.
- Assigns cases to case managers and schedules work according to priority.
- Coordinates work and vacation schedule for social service staff.
- Conducts case conferences, reviews cases, and resolves client issues.
- Audits client records and ensures compliance with Urban Pathways, regulatory and funding agency requirements and guidelines.
- Affords support and guidance to case managers in the execution of all housing related tasks.
- Documents incidents and distributes related notices within 24 hours.
- Maintains client medications, as required.
- Reviews service plans, progress notes, and other required client documentation to ensure comprehensiveness and compliance with Urban Pathways and regulatory agency requirements.
- Signs off all final documents prior to filing in client charts.
- Ensures case managers are adhering to the 360 Chart Audit on a monthly basis.
- Ensures housing documentation is completed in a timely manner for submission to housing providers.
- Manages caseload in the absence of the Case Manager.
- Completes the daily bed respite manifests as mandated by program.
- Is on call to address emergency needs on a twenty-four hour basis.
- Completes and submits relevant reports and statistics in a timely manner.
- Supervises all incident reporting and other client activities to ensure proper documentation and data entry.
- Prepares staff schedules and monitors payroll.
- Supervises the AWARDS and CARES database to ensure accuracy of client information.
- Enters data, compiles, and submits reports in a timely manner.
- Prepares and submits daily CARES reporting.
- Ensures clients are closed out of the CARES.
- Develops and maintains relationships with community services and resources.
- Participates in external meetings with funding agency and housing providers.
- Communicates verbally and in writing in a professional manner at all times.
- Facilitates staff meetings, trainings, and supervisory sessions, as required.
Qualifications
- Master’s degree in a clinical field required.
- MSW preferred.
- Minimum two years’ of clinical and supervisory experience preferred.
- Professional license (LCSW) preferred.
- Experience with homeless and mentally-ill population preferred.
