Social Service Designee

We are searching for a Social Services Designee to join our team at Harmony Palos Heights. Responsibilities

• Aid in preparing residents for their future.
• Ensure residents' social and emotional needs are met during their stay.
• Carefully coordinate resident discharges and participate in resident care conferences.
• Ensure residents are comfortable and safe based on your knowledge, skills, and attention to detail.

Requirements

• Minimum of a high school diploma or equivalent with two (2) years of experience in a Social Worker role, preferably in a healthcare setting.
• Prior Social Services Designee experience in a Skilled Nursing Facility is preferable, but not required. A bachelor’s degree in social work or a similar field and five (5) years of related experience is preferable.
