Social Service Designee

About the role

We are now hiring a Social Services Designee to join our team. This position offers a competitive salary range of $32 - $33 per hour.

Responsibilities

• Aid residents and families in adjusting to placement, illness, and the plan of care.
• Help residents and families make decisions about their treatment.
• Support residents and families during their transition and adjustment to the facility.
• Participate in resident and family council meetings as requested.
• Assist with discharge planning services.

Requirements

• Masters Degree or in process in a related field (preferably: Marriage Family & Child, Social Work MSW or related field of study).
• Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
• Ability to organize, prioritize, and meet deadlines.
• Ability to maintain confidentiality and be discreet.
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.

Qualifications

• CADAC experience with geriatric population.
• AAS or Bachelor's degree in a related field.
• Creative problem-solving skills.
• Strong communication skills.
• Sufficient computer skills to manage records and documentation.
