Social Service Designee
About the role
We are now hiring a Social Services Designee to join our team. Our mission is to provide compassionate, quality care that focuses on the unique needs of our residents and their families.
Responsibilities
- Assist each resident and family in adjusting to placement, illness, and the plan of care
- Help residents and families with wishes regarding treatment
- Support residents and families during their transition and adjustment to the facility
- Participate in resident and family council meetings as requested
- Aid in discharge planning services
Requirements
- High school diploma or equivalent
- Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals
- Ability to organize, prioritize, and meet deadlines
- Ability to maintain confidentiality and be discreet
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
Qualifications
- Nice to have: Experience with geriatric population, AA or Bachelor's degree in a related field, Creative problem-solving skills, Strong communication skills, Sufficient computer skills to manage records and documentation
