About the role
We are now hiring a Social Services Designee to join our team. Our mission is to provide compassionate, quality care that focuses on the unique needs of our residents and their families.
Responsibilities
- Aid residents and families in adjusting to placement, illness, and the plan of care.
- Help residents and families make decisions regarding treatment.
- Support residents and families during their transition and adjustment to the facility.
- Participate in resident and family council meetings as requested.
- Assist with discharge planning services.
Requirements
- High school diploma or equivalent.
- Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
- Ability to organize, prioritize, and meet deadlines.
- Ability to maintain confidentiality and be discreet.
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.
Qualifications
- Nice to have: Experience with geriatric population, AA or Bachelor's degree in a related field, creative problem-solving skills, strong communication skills, sufficient computer skills to manage records and documentation.
