Job Summary
Harmony Chicago is a Gold Seal of Approval® certified post-acute care facility that provides a wide range of services including post-hospital rehabilitation, orthopedic rehabilitation, wound care, medical rehabilitation, and tracheotomy care. Located in the culturally diverse Albany Park neighborhood of Chicago, Harmony Chicago offers a supportive and inclusive environment with Spanish and Tagalog cultural programming and multilingual support in Bosnian, Croatian, and Serbian. We are currently seeking a Social Services Designee to join our team.
Responsibilities
- Aid in preparing residents for their future
- Ensure residents' social and emotional needs are met
- Care coordination for resident discharges
- Participation in resident care conferences
Requirements
- High school diploma or equivalent with 2 years of experience in a Social Worker role, preferably in a healthcare setting
- Prior Social Services Designee experience in a Skilled Nursing Facility (preferred)
- Bachelor’s degree in Social Work or a related field with 5 years of related experience (preferred)
