Social Service Designee

About the role

We are now hiring a Social Services Designee to join our team. This is an exciting opportunity to contribute to a team committed to excellent customer service and dedicated to each individual’s unique talent.

Responsibilities

• Aid residents and families in adjusting to placement, illness, and the plan of care.
• Help residents and families with their wishes regarding treatment and support their transition and adjustment to the facility.
• Participate in resident and family council meetings as requested.
• Support admissions and assist with discharge planning services.

Requirements

• High school diploma or equivalent.
• Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
• Ability to organize, prioritize, and meet deadlines.
• Maintain confidentiality and be discreet.
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines pertaining to long-term care.

Qualifications

• Nice to have: Experience with geriatric population, AA or Bachelor's degree in a related field, creative problem-solving skills, strong communication skills, sufficient computer skills to manage records and documentation.

Pay

Schedule

This is a full-time position (Monday-Friday).
