Social Service Designee
About the role
We are now hiring a Social Services Designee to join our team. This is an exciting opportunity to contribute to a team committed to excellent customer service and dedicated to each individual’s unique talent.
Responsibilities
- Aid residents and families in adjusting to placement, illness, and the plan of care.
- Help residents and families with their wishes regarding treatment and support their transition and adjustment to the facility.
- Participate in resident and family council meetings as requested.
- Support admissions and assist with discharge planning services.
Requirements
- High school diploma or equivalent.
- Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
- Ability to organize, prioritize, and meet deadlines.
- Maintain confidentiality and be discreet.
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines pertaining to long-term care.
Qualifications
- Nice to have: Experience with geriatric population, AA or Bachelor's degree in a related field, creative problem-solving skills, strong communication skills, sufficient computer skills to manage records and documentation.

- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Attention to detail and organizational skills.

Pay
Compensation for roles varies depending on factors such as location, role, skill set, and level of experience. As required by state or local law, we provide a reasonable pay scale, including the hourly or salary range we reasonably expect to pay for this role.
Schedule
This is a full-time position (Monday-Friday).
