Responsibilities
- Provide social services to residents and their families under the direction of licensed social worker.
- Identify and address social and emotional needs, problem solve, and assist with financial issues.
- Gather information from residents/family members to support the Integrated Personal Services Assessment and MDS and its components.
- Arrange community resources, alternative placements, and assist with discharge planning as directed by the LSW.
- Attend Care Conferences as needed.
- Assist with case management tasks as required by the Department.
- Fulfill basic documentation requirements for social services.
- (After training) assist with BIMS and PHQ-9.
Qualifications
- Approved by the LSW and the facility administrator.
- STNA certification or eligibility, and experience in long-term care preferred.
- Excellent communication skills.
