About the role
The Social Care Network Coordinator is responsible for the screening, monitoring, and oversight of all Healthy Alliance Social Care Network activities within the Rescue Mission of Utica.
Responsibilities
- Conduct screenings and referrals of clients through UniteUs system.
- Monitor and track data related to Healthy Alliance services provided to clients.
- Generate and submit client data reports to supervisor.
- Cook up and process client data and documentation.
- Coordinate incoming and outgoing referrals with program staff & supervisors.
- Conduct ongoing Healthy Alliance trainings with approved staff and supervisors.
- Stay abreast of all updates and regulatory changes in the Medicaid Redesign Project on both federal and state levels.
- Build trust and rapport with clients of all Rescue Mission programs.
- Abide by all Mission, program and safety policies, procedures and guidelines.
- Attend and participate in regular meetings and trainings as requested by your supervisor and Healthy Alliance.
Requirements
- High School Diploma or equivalent.
- Minimum 2 Years experience in Human Services required.
- NYS Drivers License required.
- Familiarity with database usage and data entry.
- Experience with health management software.
- Advanced skills with Microsoft Word, Excel, and Outlook.
Qualifications
- Associates degree in related field preferred.

- $25.00 - $27.00 per hour.
Schedule
- Full time, Monday- Friday 8am-4pm 40 hours per week.
