Duties
- Conducting resident interviews to gather information about their social, emotional, and financial needs and concerns.
- Aiding in discharge planning by coordinating with healthcare providers, families, and community resources to ensure a smooth transition for residents.
- Documenting interviews, phone calls, and care conferences on a daily basis in accordance with facility policies and procedures.
- Maintaining regular communication with interdisciplinary team members, including nurses, therapists, and social workers, to ensure effective care planning and coordination.
- Adapting your daily schedule to accommodate the needs of staff, residents, and their families.
- Providing Social Services oversight completing duties as assigned for the residents of Eden Senior Care.
- Working closely with Marketing Director to coordinate referral-to-admission process.
- Communicating with insurance companies to obtain prior authorization for admissions; keeping record of insurance updates and communicating with departments included in update to ensure it is done timely.
- Aiding in the completion of admission paperwork for new residents and family members.
- Providing information to residents/families in regards to Medicare, Medicaid and other financial assistance programs available to the resident; assisting with completion of Medicaid applications as deemed necessary.
- Insure PASRR is completed to state requirements.
- Participating in resident assessment - providing background information and social history, as identified in the MDS process, in accordance with the RAI manual.
- Involving the resident and family member in planning objective and goals for the resident; developing a written plan of care to work towards meeting identified goals.
- Attending and leading Behavior Meetings and monitoring Gradual Dose Reductions (GDR) and consents for psychotropic medications according to state regulations.
- Completing timely progress notes on each resident.
- Having an open-door policy to all residents, family members, and staff to listen to their problems or needs; doing effective counseling or referrals to meet their needs.
- Serving as an advocate for the resident and his/her rights, helping the resident learn assertiveness in communicating their needs, wants, and problems to the appropriate departments and agencies.
- Participating in Monthly QAPI Committee meetings.
- Attending Stand-Up meeting daily; reporting to IDT about pay changes, care conferences, concerns, etc.
- Initiating and leading the discharge planning process; coordinating with outside resources such as: DME, Home Health, Hospice, etc. to ensure a safe transition to home or lower care setting.
- Issuing denial letters according to state regulation.
- Aiding in providing in-service education and orientation for employees regarding Resident Rights and social service needs of residents.
Requirements
- A Bachelor's degree in Social Work or a Bachelor's degree in a Human Services field including but not limited to Social Services, Sociology, Special Educations, Rehabilitation Counseling, and Psychology.
- A licensed Social Work certificate in the state of Pennsylvania, current and valid is a huge plus.
- One year of supervised social work experienced in a health care setting working directly with individuals preferred but not required.
- Able to promote cooperation between social services and other department employees.
- Must be able to be professional over the phone and in person when representing the company.
- Able to have excellent communications skills and take pride in his/her performance.
- Ability to write and give clear, concise instructions and remain calm during emergency situations.
