Social Service Coordinator

Responsibilities

• Perform assessment of the resident at admission, upon change in condition, and on regular intervals
• Create and maintain resident care plan review and amend as needed.
• Maintain thorough clinical documentation of resident contacts and progress notes.
• Cookordinate resident visits with outside services - dental, optical, and other specialized care work with the resident, family, and other team members to plan discharge.

Requirements

• Must have a minimum of one (1) year of experience in social work working directly with residents in a health care setting.
• Experience with creating comprehensive, customized care plans and discharge plans strongly preferred.
• Has the ability to professionally communicate verbally and in writing must display professionalism both in appearance and attitude.
• Must possess the ability to solve problems and make independent decisions.
• Genuinely care for and understand the elderly and disabled.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agency personnel and the public.
