Responsibilities
- Manage a full caseload of probation-involved youth.
- Attend routine team meetings with youth and families and Albany County Probation.
- Establish positive and professional relationships with youth and families.
- Track youth involvement in the program and issue incentive stipends as directed by the supervisor.
- Maintain monitored records of funds issued.
- Create coordinated service plans, to include input from youth, caregivers, subcontracted staff, and Probation.
- Help families obtain services they need by making referrals, providing transportation, accompanying them to appointments, and assisting with applications (medical, schools, and DSS).
- Maintain good working relationships with Albany County Probation/DCYF.
- Provide individual mentoring to each assigned youth, at least once per week.
- Maintain cultural sensitivity, competence, and client confidentiality.
Requirements and Education
- A high school diploma and two years of relevant experience, or an associate's degree and one year of relevant experience required.
