About the role
The full-time Service Referral Navigator serves as an expert in identifying Brooklyn-based resources for families supported through the Clinical Support Program (CSP). This position collaborates closely with the Child Protective Services (CPS) team and the CSP clinical team, making referrals and appointments for families, and facilitating trainings as needed.
Responsibilities
- Create and maintain a database of internal and community-based providers to meet the needs of families supported
- Ensure relationships with Rising Ground’s Access and Referral Department, internal programs, and community-based providers, ensuring appropriate and timely access to care
- Facilitate warm handoffs in making referrals
- Act as a conduit between Rising Ground CSP clinicians and ACS Child Protective Specialists
- Participate in all relevant internal and external meetings
Requirements
- Associates Degree minimum
- Meaningful commitment to partnering with families in a healing-centered, family-driven, anti-oppressive way
- Highly organized, self-motivated worker who can manage competing demands
- Creative problem-solver who anticipates challenges and seizes opportunities for collaboration
- Familiarity with Connections preferred
Qualifications
- Authorized to work in the U.S.
- Able to work in-person in New York City (NY)
- Able to travel to other Rising Ground sites (if required)
- Able to travel into the community
Skills
- Excellent organizational skills
- Strong communication and interpersonal skills
- Ability to manage multiple tasks and prioritize effectively
- Proficiency in database management and relationship building
