Service Facilitator (Full-Time)

About the role

We are seeking a compassionate Service Facilitator to support individuals and families through the Comprehensive Community Services (CCS) program.

Responsibilities

• Conduct needs assessments.
• Develop personalized service plans.
• Coordinate services with community resources.
• Facilitate team meetings.
• Collaborate with families and providers.
• Maintain accurate documentation.
• Advocate for clients using trauma-informed, strengths-based practices.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or a related field.
• Preferred experience working with vulnerable populations.
• Strong communication, organization, and relationship-building skills.
• Ability to manage documentation, coordinate multiple services, and work independently.
• Valid driver’s license, reliable transportation, and willingness to travel within the community.
