Service Facilitator (Full-Time)

About the role

This role involves conducting functional and needs assessments, developing person-centered service plans, coordinating services and connecting clients with community resources, facilitating team meetings and collaborating with families and providers, maintaining accurate documentation, and advocating for clients using trauma-informed, strengths-based approaches.

Requirements

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field
• Experience with individuals, families, or vulnerable populations preferred
• Strong communication, organization, and relationship-building skills
• Ability to manage documentation, coordinate services, and work independently
• Valid driver’s license, reliable transportation, and ability to travel within the community
