Service Facilitator (Full-Time)

About the role

Seeking a compassionate Service Facilitator for the Comprehensive Community Services (CCS) program to support individuals and families in accessing resources and achieving independence.

Responsibilities

• Conduct functional and needs assessments
• Develop and implement person‑centered service plans
• Coordinate services and connect clients with community resources
• Facilitate team meetings and collaborate with families, providers, and support networks
• Maintain accurate documentation and client records
• Advocate for clients using trauma‑informed, strengths‑based approaches

Qualifications and Skills

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or a related field
• Experience with vulnerable populations preferred
• Strong communication, organization, and relationship‑building skills
• Ability to manage documentation, coordinate multiple services, and work independently
• Valid driver’s license, reliable transportation, and ability to travel within the community
