Service Coordinator

About the role

The Harris Center for Mental Health and IDD is seeking a dedicated Service Coordinator to empower individuals with intellectual disabilities. In this role, you will help clients access services such as medical, psychiatric, residential, and financial support while leading person-directed planning to achieve outcomes aligned with each individual’s needs and goals. You will serve as a bridge between consumers, legally authorized representatives, providers, and collaborators, leveraging strong care-planning and assessment skills to determine current and future medical needs.

Responsibilities

• Conduct comprehensive assessments in accordance with departmental policies and procedures.
• Monitor client welfare through at least quarterly check-ins tailored to each consumer’s needs.
• Maintain data integrity by updating consumer files as required.
• Ensure timely documentation by updating records within established timelines.
• Uphold safety and health standards by complying with requirements and completing mandatory training.

Requirements

• Bachelor’s degree (B.A.) from a four-year accredited college or university in a social, behavioral, or human service field (e.g., psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, criminal justice).
• OR any Bachelor’s degree with one year of experience working with persons with intellectual and developmental disabilities.
• Minimum of one year of experience in the human services field preferred.
• Experience working with persons with intellectual disabilities preferred.
• Effective written, verbal, and organizational skills.
• Ability to work autonomously in a fast-paced environment.
• Cultural competency in communicating and collaborating with diverse consumers, families, visitors, and employees.
• Valid Texas Driver’s License and insurability under Agency policy.
• Reliable transportation.
