Senior Living Occupational Therapist- Home Health

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to perform outpatient home health visits for the Vermont Senior Living office in Williston, VT. The role serves adult and geriatric patients throughout Addison County, working as part of a multidisciplinary team to provide rehabilitative care and skilled nursing that enables clients to remain at home.

Responsibilities

• Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group home settings for adults with intellectual disabilities.
• Professionally operate at the top of licensure using current research‑based approaches to maximize functional independence for clients.
• Collaborate with home health therapy partners to transition care from home health to outpatient services, ensuring client‑centered goals are met in the living environment.
• Educate clients, families, and community caregivers on care plans to foster a team approach to maximizing functional independence.
• Conduct evaluations, treatments, and discharge planning using the outpatient EMR platform to deliver and document individualized treatment approaches.

Qualifications

• Current Vermont Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA (verified by transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Pay

Base pay: $45.00–$50.00 per hour, depending on qualifications and subject to negotiation. A $10,000 bonus is available for full‑time positions working 4 or 5 days per week.

Schedule

Flexible schedules with no holidays, no weekends, and no on‑call requirements.
