About the role
Home Health Occupational Therapist responsible for delivering personalized rehabilitation care in patients' homes.
Responsibilities
- Assess and develop therapeutic programs
- Interpret diagnostic tests
- Collaborate with healthcare teams
- Provide treatment to improve strength and function
- Review and expand retraining plans
- Recommend adaptive equipment
- Train staff and families
- Participate in care coordination, discharge planning, and community reintegration activities
- Supervise occupational therapy assistants and aides
- Document patient progress
- Ensure compliance with physician orders and guidelines
Qualifications & Skills
- Current unrestricted OT licensure
- CPR certification
- Valid driver's license
- Reliable transportation
- Preferred: minimum six months of experience and home health background
Key Duties
- Deliver therapeutic interventions
