Occupational Therapist - Home Health

Responsibilities

• Conduct patient assessments to evaluate functional status, daily activities, and occupational therapy needs in accordance with medical conditions.
• Provide services within the scope of practice, using evidence-based techniques, and coordinate with healthcare team members.
• Develop and modify therapy plans in consultation with physicians.
• Monitor, record, and report patient responses and condition changes to supervisors and physicians.
• Perform diagnostic and prognostic functional ability tests to assess patient progress.
• Deliver care in any home situation, respecting diverse backgrounds and conditions.

Qualifications & Skills

• Current occupational therapy license in the practice state.
• Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
• Preferred: CPR certification, multitasking ability, independence, strong communication, and organizational skills.
