Occupational Therapist - Home Health

About the role

This role involves providing medically prescribed occupational therapy treatments to restore function and prevent disability following injury in various settings, including home care. The therapist collaborates with a multidisciplinary team to deliver quality, compassionate, patient-centered care aligned with the organization’s mission of healing and comfort.

Responsibilities

• Assess patient needs and develop comprehensive care plans
• Implement occupational therapy interventions in patients’ homes
• Utilize appropriate principles, methods, materials, and equipment
• Communicate effectively with patients and team members
• Use office equipment such as tablets and cell phones in daily tasks

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program
• Current license in the state of practice
• CPR certification
• Strong communication, interpersonal, and professional skills
• Ability to assess needs and implement care plans
• Ability to work as part of a healthcare team
• Valid driver’s license and ability to travel within the service area
