Occupational Therapist - Home Health

Role Overview: A Home Health Occupational Therapist delivers personalized rehabilitation care in patients' homes to improve strength, mobility, and independence. Key Responsibilities: Assess patient skills, develop and interpret therapeutic programs, collaborate with healthcare teams, provide treatment and instruction, review and adapt plans, recommend adaptive equipment, supervise staff, and document patient progress. Participate in care coordination, discharge planning, and community reintegration activities to support patient recovery. Qualifications & Skills: Must have current, unrestricted OT licensure, CPR certification, valid driver’s license, auto insurance, and reliable transportation. Preferred: minimum six months of OT experience, home health experience. Additional Info: Role involves patient interaction, TB screening, and driving requirements.
