Self-Directed Community Habilitation Worker

About the role

Our dedicated front‑line staff are out in the field making it happen daily, while our administration team supports them by providing the resources and tools they need to succeed. Whether your background is in Finance, HR, IT, or Quality Assurance, you will help the agency deliver the best programs and services possible and work in a supportive, creative culture.

Location & Schedule

Location: Middletown, NY (other locations available).
Hours: Consumer‑friendly, flexible schedules.

Responsibilities

• Work with individuals to teach daily living skills and other skills identified in each consumer’s goal plan.
• Provide direct‑care, skill‑building services that support continued development of daily living abilities.
• Complete extensive documentation, articulating and recording each consumer’s progress.
• Adapt duties to the discretion of the person receiving services, following self‑directed work models.

Qualifications

• Prior experience with related populations (e.g., Intellectual Developmental Disabilities, Autism Spectrum Disorders).
• Demonstrated ability to form relationships with clients and use shared interests to promote community engagement and integration.
• Capability to advocate for others, solve problems, teach life skills, and document daily progress.
• Flexibility to support consumers’ needs and attend treatment meetings and mandatory agency trainings.
• Physical ability to perform lifting and one‑on‑one functions for specific clients.

Physical Requirements

• Ability to sit or stand at a computer for long periods.
• Comfort working in an open‑space floor plan and moving throughout the workday.
• Ability to follow consumers throughout the community and work alongside co‑workers within 3 feet.
• Capability to navigate tight spaces.
• Occasional lifting of 25 + pounds.
