Overview Of Primary Responsibilites

The Community Habilitation Workers R - Enhanced 2 will provide skill building training and supports to individuals with developmental disabilities with high levels of needs. Supports can take place in the community and/or in a certified setting.

  • Provide mentoring / teaching services to individuals with developmental disabilities to help them achieve personal outcomes and growth.
  • Teach daily living skills such as (money handling, socialization, cooking, job search, etc.).
  • Possibly perform lifting or assisting with hygiene functions for selected individuals.
  • Work on the person’s valued outcomes with the steps on how to achieve it based on the community supports and behaviors observed along with the individual’s strengths and needs.
  • Meet with the assigned individual several times per week depending on coordinated and approved hours.
  • Travel to the location assigned.
  • Attend and remain up to date on all required trainings (SCIP Training is mandatory for specific sites).
  • Document each training session in the appropriate databases and report the progress and barriers with each client.
  • Receive supervision from the Community Habilitation Supervisor.

Qualifications And Attributes

  • Prior experience with related populations, i.e. Intellectual Developmental Disabilities, Autism Spectrum Disorders.
  • Strong interpersonal and engagement skills.
  • Able to document services and understand regulations.
  • Bilingual English and Spanish speaking a plus.

Physical Characteristics

  • Must be capable to sit or stand in front of a computer for long periods of time.
  • Able to work in an open space floor plan.
  • Must be capable to move throughout the work day and follow people served throughout the community.
  • Work alongside co-workers within 3 feet.
  • Able to move in tight spaces.
  • Occasional lifting of >_25+ pounds.
