Seasonal Park Attendant - Maintenance (City Lake Park)

Duties and Responsibilities

• Provide case management for 30 to 35 clients receiving SA In-home services.
• Assist family members in developing a plan and coordinating services to maintain a disabled person in the home.
• Support the agency’s goal of preventing inappropriate placement in a long-term care facility.
• Support the legal mandate to protect disabled adults.

Knowledge, Skills And Abilities

• Accuracy required in work: Assess the client’s strengths and weaknesses and determine their needs to ensure all needs are met through program guidelines.
• Consequence of error: Incorrect assessment of a client’s condition could result in an inappropriate care plan which would not meet the client’s needs.
• Instruction provided: Expectations/outcomes of day-to-day social work practice are established by the Adult Services Supervisor and carried out with considerable independence.

Education and Experience

• Bachelor's degree in social work from an accredited school of social work; or Bachelor's degree in a human services field from an accredited college or university and one year directly related experience.
• Bachelor's degree from an accredited college or university and two years directly related experience.
