RN TRANSFER COORDINATOR - COORDINATION CENTER

Location: Menomonee Falls, WI (Woodland Prime 400 facility) – hybrid role

About the role

This position facilitates patient transfers to Froedtert Hospital, Froedtert Menomonee Falls Hospital, Froedtert West Bend Hospital, and Froedtert Community Hospitals. The individual attends emergency and non-emergency transfer requests, coordinates all transfers promptly and courteously, and collects pertinent clinical information from sending providers, including patient diagnosis, diagnostic test results, current treatment plan, past medical/surgical history, and reason for transfer.

Together with the accepting physician and sending provider, the individual responds to a wide variety of requests to determine appropriate level of care, location selection, transport mode (air/ground), and unit type. The role requires in-depth knowledge of EMTALA regulations for ED transfers and acting as a resource to key stakeholders. The individual prioritizes transfers based on patient acuity and clinical information, following established hospital workflows and protocols, often in fast-paced or critical situations.

All transfer discussions are documented in EPIC. The individual collaborates with physicians, nursing, admitting, and hospital patient flow teams to ensure timely transfers and optimal patient placement. The role also facilitates consult requests from outside providers with Froedtert & MCW physicians, participates in conference calls, documents on behalf of providers, and attends to transfer needs resulting from consultations. During overnight shifts (0000-0800), the individual may assist with patient phone triage.

Schedule

• Part-time: 0.5 FTE (20 standard hours per week)
• Shift 2: Mix of 1200-2030 or 1300-2130 on weekdays
• Every 3rd weekend rotation
• Holidays as assigned

Requirements

• Minimum of 4 years of nursing experience, with at least 3 years in acute care nursing
• Previous critical care or ED experience as an RN preferred
• Bachelor’s in Nursing required (or additional 3 years of experience, or active enrollment in a Bachelor’s program with a defined end date)
• Current Wisconsin Registered Nurse License or Multi-state Nursing License from a participating NLC (Nurse Licensure Compact) state
