About the role

Part-time, 12 hours per week. Rotating shifts including days, evenings, nights, weekends, and holidays.

The RN coordinator creates a single point of contact for transferring patients to the most appropriate Avera hospital within our system. This includes collaboration regarding each patient's unique needs and identifying the most appropriate facility. This position supports the assignment, tracking, and problem-solving to ensure timely patient placement. The RN works collaboratively with team members, nursing departments, outlying facilities, and physicians to facilitate efficient movement of patients throughout the footprint. The RN gathers clinical data to ensure continuity of patient care and appropriate patient placement, monitors all transfers, identifies potential delays in care, and provides communication between members of the care team.

Responsibilities

  • Oversees the flow of patients from outlying facilities and clinics to ensure efficient and seamless entry into the facility.
  • Prioritizes and places patients in the appropriate regional sites using critical thinking skills and following department policies and guidelines.
  • Works as a liaison with the bed placement coordinator to ensure timely transfer of patients.
  • Evaluates the transfer center process toward attainment of outcomes.
  • Collaborates with physicians, nurses, and healthcare providers, including specialists, in assessing patient placement.
  • Utilizes research and evidence-based findings in practice if appropriate.
  • Recognizes the need for knowledge and skills sufficient to integrate the use of healthcare technology and/or information management systems.

Qualifications

  • Registered Nurse (RN) with an active license in the state of practice (upon hire).
  • 1-3 years of nursing experience (preferred).

Pay

The pay range for this position is $34.50 - $51.50 per hour, with the actual pay rate dependent upon experience.
