RN Sepsis Program & Stroke Program Coordinator, 7:00 AM - 4:30 PM

About the role

The Sepsis Coordinator and Stroke Program Coordinator are both administrative positions at PIH Health, a nonprofit healthcare network serving approximately 3 million residents in Southern California.

Responsibilities

• Serve as a resource person for diverse care needs of patients, ensuring seamless navigation through the sepsis and stroke programs while prioritizing patient safety and satisfaction.
• Assist the system manager in program development, building relationships between patients and providers, and managing data collection and marketing efforts.
• Aid in developing evidence-based nursing practices for high-risk disease patient populations.
• Coordinate Joint Commission Disease Specific Certification accreditation and round on stroke-related diagnoses to ensure compliance with clinical outcome indicators.

Requirements

• Analytical skills, knowledge of Joint Commission Disease Specific Certification and Center for Medicare and Medicaid requirements.
• Effective written and oral communication skills.
• Problem-solving and organizational skills.
• Knowledge of continuous quality improvement tools, Plan Do Check Act performance improvement methodology and techniques.
• Personal computer skills and knowledge of applicable software programs.
• Ability to perform job-related activities using correct body mechanics.
• Advanced Windows and Microsoft Office applications skills preferred.

Qualifications

• 5 years experience in acute care setting.
• Required administrative experience.
• PREFERRED program development experience.

Pay

$107,120.00 - $175,032.00 annually
