Responsibilities
- Facilitates entrance of new patients into care programs.
- Provides patient education, promotes understanding of the condition, and provides ongoing continuity of care for patients.
- Responsible for coordination of pre and post hospital care.
- Serves as liaison between patient and caregivers.
- Coordinates appropriate multidisciplinary clinics.
- Performs duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the multi‑disciplinary program.
- Provides clinical support to provider and patients to assist in diagnosis and treatment of conditions.
- Collaborates with other healthcare providers in coordinating and implementing the patient's plan of care including evaluation, treatment, follow‑up and surveillance.
