Registered Nurse Navigator - ENT

About the role

Provides patient care coordination, education, and ongoing support for patients entering care programs. Acts as a liaison between patients and healthcare providers, facilitating multidisciplinary clinic coordination and guiding patients through their treatment journey.

Responsibilities

• Facilitates entrance of new patients into care programs and ensures continuity of care.
• Coordinates pre- and post-hospital care, and collaborates with healthcare teams to develop and implement patient care plans.
• Provides patient education, promotes understanding of conditions, and reinforces information through various communication methods.
• Supports clinical activities, assists physicians, and educates team members.
• Coordinates patient activities with support services, maintains follow-up systems, and ensures timely surveillance.
• Acts as a resource for patients and team members, and manages interdisciplinary activities to meet patient needs.

Qualifications

• Graduate of a Specialty Training Program (required)
• Bachelor's Degree in Nursing (preferred)
