Registered Nurse Navigator - ENT

About the role

This position involves coordinating patient entry into care programs, providing education, and ensuring continuity of care. The nurse acts as a liaison between patients and healthcare teams, facilitating multidisciplinary clinic coordination, and supporting diagnosis and treatment processes.

Responsibilities

• Facilitate pre- and post-hospital care coordination.
• Guide new patients into care programs and educate them about their conditions.
• Collaborate with healthcare providers to develop and implement patient care plans.
• Communicate with patients through various channels, ensuring timely follow-up.
• Coordinate activities with interdisciplinary clinics and support services.
• Maintain systems for monitoring patient progress and follow-up.
• Provide clinical support, nursing care, and assist with procedures.
• Educate team members and patients to promote understanding and engagement.

Qualifications & Skills

Graduate from a specialty training program (required), with a preference for a Bachelor's Degree in Nursing. Strong communication, organization, and collaborative skills are essential.
