RN Nurse Care Coordinator - Group Home

About the Role

A Home Health RN position focused on providing skilled nursing and home-based care to individuals in group homes across multiple locations in New Jersey.

Responsibilities

• Assess patients, document progress, and report changes to physicians
• Educate caregivers and support professionals on care plans, medication, therapy, diet, and exercise
• Coach staff on patient care, medication management, and home safety
• Coordinate care and communicate with families, staff, physicians, and hospitals

Requirements

• Active RN license in New Jersey
• Minimum of 2 years nursing experience, preferably with developmentally challenged individuals or in group home settings
• CPR certification (within 60 days of hire)
• Knowledge of home health and group home regulations
